Testosterone should be avoided or used with caution in:
- Women with high cardiometabolic risk or active liver disease (due to a lack of safety data).
- Where there is a history of breast cancer –off label exceptions may be agreed in women with intractable symptoms not responding to non-hormonal alternatives. These cases should be discussed with a menopause specialist and/or the breast or oncology team.
- Competitive athletes. Care must be taken to maintain levels well within the normal female physiological range.
- Women with upper normal or high baseline total testosterone levels.
- Pregnant or breast-feeding women.
